Maybe Sustained Attention Is Not Just Executive Function That Lasts Longer

We have repeated the same ADHD solutions for years. A new genetics study suggests the problem may be more complicated than our toolkits admit.

I have always been wary of explanations that arrive with a ready-made toolkit attached.

In ADHD coaching, executive dysfunction has often been treated as the master explanation. Difficulty starting, organising, prioritising or completing a task is met with familiar solutions: break it down, define the next action, block the time, externalise the plan, reduce the working-memory load.

These tools are not without value, I use many of them. The problem is what happens when we present them as though they explain everything.

Clients often return with an awkward response:

I did that. It still did not work.

They made the list. They blocked the time. They reduced the task to one manageable step. Sometimes they even started.

And then they lost it.

Not because they forgot the task mattered. Not because the plan was unclear. Not because they failed to understand the consequences.

They simply could not remain connected to it.

As a coach, this was one of the reasons I grew uneasy with purely executive-function explanations. They often described the visible failure without accounting for the forces underneath it.

Emotion dysregulation supplied part of what was missing.

A task might look simple on paper while carrying shame, threat, resentment, anticipated criticism or the memory of previous failure. The person was not merely trying to write a report. They were trying to write while managing what the report had come to represent.

Once emotion was included, many apparent failures of planning became easier to understand.

But that explanation brought another temptation.

If emotion was the barrier, perhaps meaning was the answer.

Connect the task to purpose. Clarify the values behind it. Make the outcome personally significant. Find the reason compelling enough to pull attention into place.

Again, there is some truth in this. But purpose can become another panacea.

It can quietly create the idea that meaningful tasks should hold attention, and that a failure to sustain attention means the person has not found the right motivation, emotional connection or reason to care.

That does not fit what many clients describe.

They may care deeply, understand the consequences. even feel intense urgency, or simply organised the task, protected the time and begun the work.

Yet their attention still fails to remain attached.

And once it moves, finding the way back can be far more difficult than the original act of starting.

This raises a question that our prevailing narratives do not answer well:

What if difficulty sustaining attention is not merely the result of poor executive control, weak motivation or unresolved emotion?

recent study in Molecular Psychiatry gives us reason to take that possibility seriously.

A distinction hiding inside a genetics study

The researchers examined the genetic relationships between several cognitive processes.

On the executive-function side, they looked at measures such as:

  • working memory;

  • processing speed;

  • response selection.

On the sustained-attention side, they examined:

  • vigilance;

  • the likelihood of attentional lapses;

  • the cost of recovering after a lapse.

The results were best explained by two partly distinct genetic factors: one related to executive function and another related to sustained attention.

The two factors were not completely independent, but neither did they collapse neatly into a single general capacity.

Most importantly for ADHD, both showed independent genetic associations with ADHD liability. That suggests the relationship between ADHD and cognition may involve both executive functioning and attentional stability, rather than one simply being a longer version of the other.

The study does not prove a new theory of ADHD.

It does not tell us exactly what is happening when someone loses the thread of a report, abandons an email halfway through or realises that an hour of protected work has dissolved into unrelated activity.

But it gives us permission to consider an alternative view.

Perhaps sustained attention is not merely executive function continuing over time, but rather that remaining connected to a task is itself a distinct cognitive demand.

The prevailing narratives explain the beginning better than the middle

Most ADHD strategies are designed around getting started.

Find meaning —> Create urgency —> Clarify the first step —> Use a timer —>

Block the time —> Make the task smaller —> Work beside someone.

These approaches can be useful because task initiation is genuinely difficult.

But starting a task and sustaining a task are not the same event.

Opening the document is not the same as becoming attentively established inside it.

Writing the first sentence is not the same as remaining connected to the argument.

Sitting inside a protected time block is not the same as maintaining task contact.

The usual assumption is that once initiation occurs, momentum should take over. If it does not, we conclude that the task was not meaningful enough, the structure was not clear enough or the person did not commit strongly enough.

That conclusion can become another route back to self-blame.

The client tried the strategy. The strategy did not hold. Therefore, the client must not have tried properly.

There is something almost impressive about the efficiency with which every failed intervention can be converted into evidence against the person.

Meaning is not an attentional maintenance system

Meaning can help a person choose a task.

It can strengthen willingness, reduce internal conflict, and it can answer:

Why should I do this?

But it does not necessarily answer:

What keeps my attention attached to this once I have begun?

A task can be deeply meaningful and still be almost impossible to sustain.

In fact, importance may sometimes make attention less stable rather than more stable.

A meaningful task may carry more pressure, more anticipated judgement and more fear of failure. Attention becomes captured not by the work itself, but by what the work means.

The person is no longer writing the report.

They are monitoring whether the report proves their competence.

They are imagining the reaction of the manager.

They are predicting the consequences of getting it wrong.

They are rehearsing the future shame of failure.

The task remains important. It may be too important.

Attention is sustained, but around threat rather than action.

This is why increasing meaning can sometimes deepen paralysis. It adds emotional weight without necessarily improving attentional stability.

Purpose can help with orientation.

It is not a guarantee of persistence.

Time blocking protects time, not attention

Time blocking is another strategy that is frequently treated as more powerful than it really is.

A time block creates a protected opportunity for work.

It does not guarantee that attention will remain inside that opportunity.

A person can reserve ninety minutes for a task and still spend most of it rereading, drifting, researching tangential points, reorganising the folder structure or trying to remember what they were doing five minutes earlier.

The time block has technically worked.

The time exists.

Attention simply has not respected the booking.

This does not make time blocking useless. It means we should stop pretending that a calendar can perform cognitive regulation.

A protected hour may be necessary.

It may not be sufficient.

The problem may be task contact

One way to rethink sustained attention is through the idea of task contact.

Task contact is not simply knowing that the task exists. It is the active cognitive connection between the person and the next part of the task.

When task contact is strong, the person knows what they are doing, why the current step matters and what follows from it.

When task contact weakens, the task becomes abstract.

The person may still know they are supposed to be working on the report, but they no longer feel inside the logic of the report.

The work becomes something they are looking at from the outside.

This may help explain why some tasks are so difficult to resume.

The document remains open, but the cognitive path back into it has vanished.

The person must reconstruct:

  • what they were trying to say;

  • which decision they had reached;

  • what evidence they were using;

  • what the unfinished sentence was meant to become;

  • what the next meaningful action should be.

“Get back to work” sounds simple only when we ignore how much work it takes to get back.

Losing attention may not involve a choice

Much productivity advice assumes there is a moment of decision.

You are doing the task.

You become tempted by something else.

You choose whether to remain focused.

This makes distraction sound like a failure of inhibition.

But attentional drift often does not arrive as a clearly marked fork in the road.

It is more like gradual weather.

A relevant thought becomes a related thought. The related thought becomes a useful search. The search produces an unexpected detail. The detail opens another thread.

At no point does the person consciously decide:

I will now abandon the task I care about.

They simply discover, some time later, that the task is no longer active.

That difference matters.

A strategy based on resisting temptation assumes the person notices the transition while it is happening.

Often they do not.

The intervention may therefore need to focus less on self-control and more on early detection.

Not:

How do I stop myself from switching?

But:

How do I notice that the original task is fading before it disappears completely?

The cost of returning may be the real problem

The paper’s treatment of lapse recovery is particularly interesting.

The researchers examined what they described as lapse recovery cost: the performance cost associated with re-engaging after an attentional error.

This measure did not fit neatly into either the executive-function or sustained-attention factor.

That awkwardness may be useful.

Recovery appears to sit at the boundary between maintaining attention and rebuilding control after attention has been lost.

In everyday work, returning to a task may involve at least three separate demands.

First, attention must disengage from whatever currently holds it.

Second, the earlier task state must be reconstructed.

Third, the person must tolerate the emotional response to having drifted.

That final demand is easy to miss.

A small lapse can trigger a large judgement.

I have done it again.

I have wasted the block.

This should not be so difficult.

I will never finish.

Now there is no point continuing.

The original attentional lapse may have lasted two minutes.

The emotional collapse can consume the rest of the afternoon.

This is why return is not merely a cognitive act - it is also an emotional one.

The person must find the task again while managing the shame of having lost it.

Emotion still matters, but it is not the whole explanation

None of this makes emotion dysregulation less important.

Emotion can shape:

  • what captures attention;

  • what attention avoids;

  • how effort is experienced;

  • how quickly a lapse becomes threatening;

  • whether returning feels tolerable;

  • whether one disruption becomes a full abandonment.

But emotion does not need to be the sole cause of the attentional instability.

That distinction matters.

A client may not be avoiding the task because it lacks meaning.

They may not be resisting because of unresolved threat.

They may not be failing because the plan is too vague.

They may simply struggle to maintain the task as a stable object of attention.

Emotion can then amplify the difficulty.

The lapse produces frustration.

The frustration produces self-criticism.

The self-criticism makes re-entry harder.

Now a primary problem of attentional instability becomes entangled with a secondary problem of emotional cost.

This is often how ADHD difficulties become layered.

The original mechanism is difficult enough., the meaning we attach to it makes it worse.

Perhaps we should stop treating lapses as failures

Most productivity systems are built around an ideal of uninterrupted focus.

The successful person begins, remains on task and finishes.

The distracted person is encouraged to create a better environment, use stronger discipline or identify a more powerful motivation.

But for many people with ADHD, lapses may be unavoidable features of working attention.

The useful question may not be whether attention will drift.

It may be:

  • How quickly is drift noticed?

  • How much task context is lost?

  • How far does the person travel before recognising the lapse?

  • How much emotional punishment follows?

  • How easy is it to reconstruct the point of return?

This shifts the goal from perfect continuity to recoverable continuity.

A person who loses focus six times and returns within thirty seconds may work more effectively than someone who sustains focus for twenty minutes and then disappears from the task for the rest of the afternoon.

The number of lapses may matter less than the cost of each one.

Why common strategies sometimes fail

This alternative view helps explain why sensible interventions can produce inconsistent results.

“Make the task meaningful”

This may strengthen intention without stabilising attention.

“Break it into smaller steps”

This may reduce executive complexity without preventing the task from fading from awareness.

“Use a timer”

A timer marks the passing of time. It does not necessarily restore the lost cognitive thread.

“Remove distractions”

This may reduce external competition while leaving internal drift, rumination or associative thinking untouched.

“Just work for five minutes”

This works when five minutes creates attentional momentum. It fails when the task remains unstable throughout those five minutes.

“Write a clear to-do list”

A list preserves the existence of the task. It may not preserve the mental route into it.

These are not bad strategies. They are strategies aimed at different mechanisms.

The problem arises when we use them as universal solutions and then blame the client when the mechanism does not match the difficulty.

External structure may need to preserve continuity, not just intention

If sustained attention is partly distinct from executive control, then support may need to focus on maintaining and restoring task contact.

That could mean:

  • leaving a precise restart note before every pause;

  • ending work in the middle of a clear next step;

  • recording the current line of thought rather than only the task title;

  • using short orientation prompts during work;

  • speaking the next step aloud;

  • working alongside another person;

  • keeping visible evidence of the task’s current state;

  • building deliberate return points into the work session.

A note saying “Finish report” is almost useless after attention has drifted.

A note saying “Next: compare the second finding with the limitation on page eight, then explain why the causal claim is too strong” recreates the doorway into the work.

That is not merely task management.

It is attentional continuity made external.

Body doubling may be doing more than creating accountability

Body doubling is often explained through social pressure.

Someone else is present, so I am more likely to behave.

That explanation may be too narrow.

Another person may help keep the task context active.

Their presence may reduce the likelihood that internal thought becomes the entire attentional environment. They may provide temporal structure, low-level stimulation and a stable point of orientation.

The person does not necessarily work because they fear being judged.

They may work because the task is less likely to disappear.

This would explain why silent co-working can help even when the other person is not monitoring, prompting or checking progress.

The second person may act less like a supervisor and more like an external anchor.

This is not an argument against executive-function models

Executive-function difficulties remain central to ADHD.

Planning, working memory, inhibition, sequencing and self-monitoring all matter.

Emotion regulation also matters. Meaning matters. Context matters.

The point is not to replace one totalising explanation with another.

It is to stop treating sustained attention as though it were merely executive function stretched across time.

The new study does not prove that everyday persistence problems arise from a separate sustained-attention mechanism. Genetic covariance is not lived experience, and online cognitive tasks are not equivalent to writing a report under pressure.

The study also has important limitations. Its sample was restricted to participants with European-like genetic ancestry. It was enriched for mood disorders. The sustained-attention factor was less precisely measured than the executive-function factor. The findings require replication in larger and more diverse samples.

But the paper supports a more granular view.

It suggests that our ability to execute cognitive control and our ability to maintain a stable attentional state may overlap without being identical.

That distinction fits something many people with ADHD have been trying to explain for years:

I know what to do.

I know why it matters.

I have made the time.

I have even started.

I just cannot keep hold of it.

Three questions before reaching for another strategy

This does not mean abandoning practical tools. It means asking better questions before prescribing them.

1. Where does contact with the task actually break?

Not whether the task was started, but where it stopped being mentally present.

Was the person unable to settle into it?

Did attention drift after a few minutes?

Did a minor interruption erase the thread?

Did the task remain visible while its internal logic disappeared?

The more precisely we locate the break, the less likely we are to reach automatically for another planning tool.

2. What makes the task difficult to hold, rather than merely difficult to begin?

The answer may involve boredom, ambiguity, emotional threat, low stimulation, internal distraction, fatigue or the absence of immediate feedback.

But it may also be that the person’s attention simply does not remain stable enough for the task to keep its place.

That possibility should not be treated as a failure to find sufficient meaning.

Sometimes the task matters. The attention still moves.

3. What would make returning less expensive?

A useful intervention may not prevent every lapse.

It may preserve enough of the task that the person can return without rebuilding the entire mental state.

That could mean leaving a clear restart cue, recording the unfinished thought, keeping the next decision visible, or using another person as an attentional anchor.

The goal is not flawless concentration.

It is continuity that can survive interruption.

These questions do not produce a neat formula. That is probably their value.

They resist the urge to turn every difficulty into another standard toolkit and ask us to pay closer attention to what the person is actually experiencing.

Because sometimes the problem is not that they do not know what to do.

It is not that they do not care.

It is not even that they cannot begin.

It is that the task will not stay with them—and once it is gone, finding it again is harder than we have allowed for.

A different starting question

Perhaps the better question is not:

How do we make this task meaningful enough to sustain attention?

Or:

How do we create enough discipline to stay with it?

Perhaps it is:

What allows this person to maintain contact with the task, notice when that contact weakens, and return without having to reconstruct everything from scratch?

That question moves us away from moral explanations.

It also moves us beyond a productivity culture that assumes intention should naturally become persistence.

For some people, intention gets them through the door.

The real challenge is keeping the room from disappearing.
